Dermal piercings are a response to a popularity of 'skin diver' piercings that were popularized in the UK. Dermals are typically two pieces of jewelry (the boot inside the skin and the topper that is seen) and typically preformed with an O-needle or chamfer needle. This is because the biopsy punch tool was outlawed in many states for piercers to carry in shop.
Dermals can technically be placed almost anywhere but all with differing degrees of success. You COULD technically do one on your butt cheek but it probably wouldn't fare very well in the long run and may end up contracting a illness or infection.
The jewelry most commonly used is a Sean Dowdell design that contains two holes in the boot. The bigger hole being the toe end and the smaller opening being the heel end. This allows for skin and scar tissue to grow on the inside to help it not fall out. However these piecing types are considered more advanced and are more than likely not permanent as they can get pulled out, reject, or have career ending (for the piercing) consequences much easier than others. Be sure to keep these clean.
Typically you can get the top switched out as early as the first month after healing but is usually not advised if we want the piercing to stay longer. The less irritation this one gets, the better.
If a dermal seems to be coming more out of the skin, its typically best to get it removed to reduce the chance of scarring.
